If you were unwell, and unable to speak for yourself, do those you are close to know your wishes for health care and personal decisions?

Advance care planning is the process of thinking about, talking about, and documenting your wishes for health care if you are unable to speak for yourself.

This session will provide information on Advance Care planning and the importance & role of Personal Directives.

All participants will be provided free materials

Presenter: Alexandra Kushliak BA, BSW.RSW, is an Educator Consultant with Advance Care Planning and Goals of Care, Calgary Zone, Alberta Health Services
